Exclusion Criteria for SILS

In a 2013 study published in the Journal of Minimal Access Surgery, Reinhard Mittermair listed the following exclusion cases in which single incision sleeve gastrectomy may not be feasible:2

  • Excessive sweating after eating
  • Body mass index (BMI) exceeding 50 (and with excess abdominal fat)
  • Height exceeding 180 cm
  • Patients with prior:
    • Surgery of the upper abdomen
    • Surgery for repairing umbilical hernia
  • Individuals who are addicted to drugs or alcohol
  • High-risk surgical patients

Disadvantages

  • Small incision tends to crowding of trocars in a small surgical area
  • Challenge of handling an enlarged liver and excess abdominal fat (however, in the research study by Reinhard Mittermair3 it was suggested that the liver can be retracted by mobilizing the stomach)
  • Restricted instrument triangulation (resulting in clashing of the instruments) for the surgeon; this limitation can, however, be overcome with prebent instruments as explained by Prasanth P Rao et al. in their 2011 research study4
  • Restricted maneuverability
