What is Single Incision Laparoscopic Sleeve Gastrectomy?

  • After first being described for appendectomy and hysterectomy in 1992 by Pelosiet al., the application of single incision laparoscopic surgery (SILS) has also been extended to vertical sleeve gastrectomy.
  • One incision SILS vertical sleeve involves making a single transumbilical incision (in the belly button).
  • Earlier approach to SILS involved making an incision in the upper abdomen. However, it was felt that cosmetic outcomes would be better with the transumbilical approach.1
  • Often, there is no visible trace left to know that a surgery even took place.

Benefits of Single Incision Procedure

  • Single Incision Laparoscopic Sleeve GastrectomyPort incisions are limited to just one place
  • Muscle penetration is avoided which minimizes pain
  • Lower trauma (as the umbilicus is the thinnest part of the abdominal wall) implies less pain after surgery
  • Compared to traditional laparoscopic surgery, this also results in:
    • Reduced rate of wound complications
    • Better cosmetic outcomes
    • Less use of analgesics (pain medication)
    • Faster recovery

SILS vs. Multiple Port Laparoscopic Surgery

Basis of Difference

Multiport Laparoscopic Surgery

Number of Incisions

5 to 7

Only 1 umbilical incision (a second incision may be required to place a liver retractor)

Number of Trocars placed through the Incisions

5 to 7 trocars

About 3 trocars

Mean operative time

Longer than multiport approach (about 120 minutes)

Shorter than SILS (about 90 minutes)

Site for incision(s)

Umbilicus (belly button)

Virtually scar-free as the incision is made in the belly button

More scarring than SILS

Excess Weight Loss

Comparable to traditional lap. surgery

Comparable results2

Exclusion Criteria for SILS

In a 2013 study published in the Journal of Minimal Access Surgery, Reinhard Mittermair listed the following exclusion cases in which single incision sleeve gastrectomy may not be feasible:2

  • Excessive sweating after eating
  • Body mass index (BMI) exceeding 50 (and with excess abdominal fat)
  • Height exceeding 180 cm
  • Patients with prior:
    • Surgery of the upper abdomen
    • Surgery for repairing umbilical hernia
  • Individuals who are addicted to drugs or alcohol
  • High-risk surgical patients

  • Small incision tends to crowding of trocars in a small surgical area
  • Challenge of handling an enlarged liver and excess abdominal fat (however, in the research study by Reinhard Mittermair3 it was suggested that the liver can be retracted by mobilizing the stomach)
  • Restricted instrument triangulation (resulting in clashing of the instruments) for the surgeon; this limitation can, however, be overcome with prebent instruments as explained by Prasanth P Rao et al. in their 2011 research study4
  • Restricted maneuverability

However, there is constant effort to overcome these limitations. Also, the surgical limitations of SILS far outweigh its benefits.

Why is Cost Lower?

The lower cost of minimally invasive gastric sleeve in PVR, Mexico is the primary lure to have the surgery south of the border. But this sometimes deters people who believe the myth that the price is so low because of shoddy medical care and obsolete technology and facilities. This is not the case. The procedure is able to be offered at such affordable rates for a number of reasons:

  • The biggest factor is the low cost of living in Mexico, something which Americans have long crossed the border to take advantage of. Everything from hotels, food, tourist activities and prescription drugs simply cost relatively less in Mexico. Healthcare does as well.
  • Doctors in Mexico do not have as much student debt obligations as their peers in the US or Canada. Because of this, they are able to keep their fees low.
  • The leading bariatric surgeons performing SILS in Vallarta and elsewhere in Mexico also do not have to pay the outrageous malpractice fees that those in the United States do. They also do not have to hound insurance companies for reimbursements that get excused away, which means they don’t raise fees of one incision VSG in Vallarta to make up for this expense.
